CHRONIC CARE MANAGEMENT (CCM) involves managing a patient’s multiple chronic conditions expected to last at least 12 months. CCM services are designed to provide comprehensive support, including non-face-to-face interactions, to help patient effectively manage their chronic conditions.

REMOTE PATIENT (PHYSIOLOGIC) MONITORING (RPM) allows a patient to collect their own health data, like blood pressure, weight, and glucose levels, using connected medical devices that automatically transmits the data to their health care provider. The provider then uses this data to treat or manage (remotely monitor) the patient’s condition.

REMOTE THERAPEUTIC MONITORING (RTM) uses technology to remotely engage patients at home in activities to improve musculoskeletal/balance, respiratory function, and cognition, thereby preventing cognitive decline and falls. Our acclaimed RTM platform, BrainHealth+ is the most robust and engaging platform available and is a completely turnkey system in which patients can self enroll and be completely supported by our clinical team.
